Our responsibilities

Lakehills Dental Center is required by law to protect the privacy of your protected health information, provide this notice, follow the notice currently in effect, and notify you when a breach requires notification. When another applicable law provides greater privacy protection than HIPAA, the practice will follow the more protective requirements.

How health information may be used and disclosed

Treatment

We may use and share health information to provide, coordinate, or manage your dental care, including with specialists, physicians, dental laboratories, pharmacies, or other providers involved in treatment.

Payment

We may use and disclose information to bill for services, obtain insurance benefits, request prior authorization, process claims, or collect payment.

Health care operations

We may use and disclose information to operate the practice, evaluate and improve care, train personnel, conduct audits, manage billing, obtain professional advice, and perform other permitted administrative activities.

Appointments and care communications

We may contact you about appointments, treatment, follow-up care, or payment using available communication methods, subject to applicable law and reasonable confidential-communication requests.

Family and others involved in care

We may share information relevant to another person’s involvement in your care or payment when you agree, have an opportunity to object and do not object, or the law otherwise permits.

Business associates

We may share information with service providers performing functions for the practice. When required, those providers must agree to safeguard the information and limit its use and disclosure.

Subject to legal requirements and safeguards, information may also be used or disclosed for legal compliance, public-health activities, reports of abuse or neglect, health oversight, judicial proceedings, authorized law-enforcement purposes, prevention of serious threats, duties of coroners and funeral directors, organ donation, permitted research, special government functions, and workers’ compensation.

Information requiring additional protection

Additional federal and Texas protections may apply to certain substance-use-disorder records, HIV-related information, mental-health records, genetic information, and other sensitive information. Lakehills Dental Center will obtain additional consent or authorization when required and comply with applicable restrictions.

Uses requiring written authorization

Written authorization will be obtained when required, including for most uses and disclosures of psychotherapy notes, marketing communications requiring authorization, the sale of protected health information, and other uses or disclosures not described in the notice unless otherwise permitted or required by law. Appropriate written authorization will be obtained before using identifiable patient photographs, videos, testimonials, or treatment information for public advertising, website promotion, or social media. You may revoke an authorization in writing; revocation does not affect actions already taken in reliance on it.
